In the Subject field, write the Woodbury Police Department Blotter number you were provided at the scene of the accident.
In the Message field, write the Date of the accident, Location of the accident, and the Officer that handled your accident.
Note: There are three options for receiving your copy. It can be emailed to the address you provide, you can provide a phone number in the Message field and will we contact you when it is ready to be picked up in person or it can be mailed to you.
If you choose to have it mailed to you, please provide a complete mailing address in the Message field.
